Please fill out online forms prior to coming in for your appointment, this allows for the full treatment time

Parking/Entry: On driveway, or parking lot next door (dentist). Entry through side door (closest to driveway) and down the stairs

Not wheelchair accessible, there is a full flight of stairs to get to treatment room

Direct billing/payment methods: Direct billing to most major insurance companies, please make sure you submit your insurance info prior to your appointment if you prefer direct billing. It is your responsibility to pay any remaining balance, I accept cash, etransfer & debit/credit with tap only.

Cancellation policy: Please allow at a minimum of 24 hours notice if you can’t make it for your appointment time. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a 50% cancellation fee
