Please fill out online forms prior to coming in for your appointment, this allows for the full treatment time

Parking/Entry: On driveway or street, please do not block sidewalk/road or fire hydrants. You can enter through the garage once it’s open.

Not wheelchair accessible, there is a full flight of stairs to get to treatment room

Arrival: You can text 905-440-0912 when you arrive, Nicole will open the garage and come get you approximately 5 mins before your appointment start time.

Direct billing/payment methods: Direct billing to most major insurance companies, please make sure you submit your insurance info prior to your appointment if you prefer direct billing. It is your responsibility to pay any remaining balance, I accept cash, etransfer & debit/credit with tap only.

Cancellation policy: Please allow at a minimum of 24 hours notice if you can’t make it for your appointment time. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a 50% cancellation fee
